Religiosity and Sexuality Questionnaire

Participant Information Sheet

You are being invited to participate in a study that will investigate the link between religiosity and experiences of high-risk sexual activities in young adults.

This study is being conducted Dr. Patricia Weerakoon, Program Coordinator of the Graduate Program of Sexual Health and Dr. Gomathi Sitharthan, Senior Lecturer in Psychology and Deputy Academic Coordinator of the Graduate Program in Sexual Health.

If you choose to participate in this study you will be completing an online questionnaire that takes approximately 20 minutes to complete.

Participating in this study is completely voluntary - you are not under any obligation to consent. If you do participate, you are to withdraw your consent and to discontinue your participation at any time without prejudice. Submission of your completed questionnaire will be taken as your consent to participate in the study and, since no identifying information will be collected, once you have submitted your questionnaire, your responses cannot be withdrawn.

All aspects of the study, including results, will be strictly confidential and only the researchers will have access to results. There will be no identifiable information on the questionnaire, and once submitted it will be impossible to know who completed it. A report of the study may be submitted for publication, but individual participants will not be identifiable in such a report.

There are no direct, tangible benefits from participating, however the answers you provide will contribute to important scientific research on sexual health and will assist in increasing the knowledge base in the field of religiosity and sexual health.
