First Time Users
Complete this survey for the first time 10 days after the start date listed on your EAD card.
If you have not received your EAD card, complete this survey for the first time 10 days after the start date of OPT listed on the 3rd page of your I-20.
All Users
You also must return and complete this survey within 10 days after any of the changes described below.

You must submit a copy of your EAD card (OPT card) to the International Center upon receipt from the government. Failure to do so may result in a cancellation of your OPT status. A copy of your EAD card may be submitted via:

The Department of Homeland Security (DHS) requires all F-1 students to report the following information during their post-completion OPT period:

Change in Name
Change in Address (see below)
Change in Employer
Interruption of Employment
Permanent Departure from the U.S.
Change of Immigration Status

U.S. Address Changes in MyIIT


Do not use this survey for address changes.

To keep your MyIIT account active, you must log-in at least once every 5 months. If you cannot log-in, contact the Office of Technology Services at http://ots.iit.edu/.

Address changes must be made in MyIIT within 10 days of moving. The Local address type in MyIIT must be your U.S. address. Generally, you should have a U.S. address in the same state where you are working.

MyIIT Instructions


To update your LOCAL and / or PERMANENT INTERNATIONAL address types as required by the Department of Homeland Security for F-1 and J-1 students follow these steps:

1) Login to your MyIIT portal

*If having trouble logging in, please read the support information on the login page

2) Click on the "Academics" Tab

3) On the right side, click on “Banner Self Service”

4) Select “Personal Information”

5) “Update Addresses and Phones”

6) Select Address to update

-  Select LOCAL Address to update your Current US address (Include your Dorm or Apt number if you have one)

-  Select PERMANENT INTERNATIONAL to update your address in your home country (NOT in the United States)

 

You may have to add one of both of these address types if you do not see them.  Just scroll to the bottom of the screen and select Local and or Permanent International from the drop down list.


7) Click on Submit when completed.
