Financial Health Survey

Please respond to the following questions as completely as possible. You may want to have a few financial records on hand, such as your 2008 tax return, checkbook register and credit card statements. Use your best guess on each question. It should take 15 to 20 minutes with your financial records available and slightly longer without them. At the end of the survey, you will be provided with links to financial education resources that address your financial needs.

This survey is provided to disseminate information and for academic research purposes.  Results are being calculated anonymously.  No identifying information is being collected during this survey.
