ASSESSMENT AND TRIAGE OF SUICIDAL ADOLESCENTS

This survey is your chance to give voice to the concerns, needs, and strengths of the primary care community related to working with adolescent patients with behavioral health problems in general, and suicidality specifically. Your responses will be summarized and presented to advocacy groups, insurance companies and state officials to garner more support and resources for the primary care community. Your responses will be anonymous. Therefore, don't give the "right" answers, but rather answers that reflect what you actually think and do in practice. We appreciate your support for this project and will provide feedback through your state professional organization.

All questions focus on adolescent patients who are 13 to 19 years old.
