WV Statewide Afterschool Network Afterschool Program Survey

The following survey is designed to gather information from directors of programs that offer organized activities for at least two hours between the end of the school day and 6 PM. (Summer programs may be offered in addition.) These programs should serve students between grades K and 12. The form should be completed for one physical location only. Please complete the survey if your program meets these criteria and you serve as the program director or the site coordinator. For all fields that require input of a number, use a numerical value as it will help in our analytical purposes (e.g. 5 or 11, not five or eleven).The information you provide will contribute to accurate knowledge about out-of-school time programs in West Virginia.
