Tall Building Motion Survey

You are invited to participate in a study of human perception of motion in tall buildings. This project is being overseen by researchers at the University of Notre Dame, Skidmore Owings and Merrill LLP, and the Boundary Layer Wind Tunnel Laboratory at the University of Western Ontario (referred to herein as “Project Team”). The Project Team hopes to learn the level of wind-induced motion that is acceptable to occupants of tall buildings. You are agreeing to participate in this program by completing the following survey, which features questions on the nature of the motion you experienced and its effect on you. Any information that is obtained in connection with this study and that can be identified with you will remain confidential and will be disclosed only with your permission. We plan to disclose your answers to the questions but no information concerning your identity. Your responses will be used by the Project Team for only research purposes and may be reported in future publications. Completion of this survey implies your agreement with these terms. Your decision whether or not to participate will not prejudice your future relations with the Project Team. If you decide to participate, you are free to withdraw your consent and to discontinue participation at any time without prejudice. If you have any questions, please email Dr. Tracy Kijewski-Correa (tkijewsk@nd.edu) of the University of Notre Dame and she will be happy to answer them. YOU ARE MAKING A DECISION WHETHER OR NOT TO PARTICIPATE. BY COMPLETING THIS SURVEY, YOU INDICATE THAT YOU HAVE DECIDED TO PARTICIPATE HAVING READ THE INFORMATION PROVIDED ABOVE.

PLEASE CONTINUE WITH THIS SURVEY ONLY IF YOU HAVE EXPERIENCED THE SENSATION OF A BUILDILNG MOVING DURING A WIND STORM.
