The Michigan Department of Human Services (DHS) is surveying foster parents and relative caregivers, such as you, who recently had children, who were new to the foster
care system, placed in their homes for at least 30 days. The information being sought concerns receipt of Medicaid cards for these children, as well as the availability and quality of medical, dental and mental health services in communities.
The survey is made up of a maximum 17 questions (as you may not be required to answer all of them) and should only take a couple of minutes to complete. Thank-you for taking time to complete
