eClinicalWorks would like to get a better sense of your views on Meaningful Use.  This survey is to provide insight to eClinicalWorks on the market perception and readiness for Meaningful Use. 1	Do you qualify for the incentives based on the definition of eligible providers under the CMS proposed ruling? I qualify for this incentive under the proposed ruling by CMS  I do not qualify for this incentive program under the proposed ruling by CMS I am not sure if I qualify

2	Are you committed to becoming a meaningful user? I am eligible for the incentive AND I am committed to becoming a meaningful user I am not eligible for the incentive BUT I am still committed to becoming a meaningful user I am not committed to becoming a meaningful user based on the Meaningful Use definition  I am not familiar enough with the Meaningful Use definition to commit

3	Do you view Meaningful Use primarily as a Centers for Medicare and Medicaid (CMS) incentive program for EHR adoption? I believe that Meaningful Use is primarily targeted as a financial incentive toward providers that are eligible under the CMS proposed ruling  The incentive provides financial assistance for adoption, but I believe Meaningful Use has a broader impact than the  incentive dollars I am not familiar enough with Meaningful Use to understand the impact

4	Do you believe that the nationwide adoption of EHRs with providers exhibiting meaningful use will have a positive impact to the care provided to patients? Yes No I am not familiar enough with Meaningful Use to answer
