The Stoughton Public Schools would like to thank you for your participation in this survey.  As a nation, there has been a tremendous focus on health and nutrition,
including the impact of these factors on student success and achievement.  In order to better serve our students and the community, this survey has been developed to help shape the District's Wellness Policy, specifically the nutrition component.  Your  participation
