1. All entries must be submitted via this official Web form or by a printed hard copy of this ballot. Responses received in any other format will NOT be accepted.
2. All information received electronically by the Auburn Chamber will be for internal review purposes only. This information will not be sold, shared, or otherwise conveyed to other individuals or organizations for commercial purposes. Anonymous comments may be used for the purpose of contest marketing and awards presentation.
3. Nominated businesses MUST be located in the town of Auburn, Massachusetts.
