The California Hepatitis Alliance (CalHEP) is developing a referral guide to help individuals identify hepatitis B & C resources across California. With your help, the referral guide will include information on where to find screening, support/education groups, syringe exchange, referrals to treatment, patient navigators, and vaccination. This searchable guide will initially be online, followed by print copies for populations without internet access.

We are asking you to complete this short survey (approximately 5 minutes) so we can determine whether your organization provides the above services, for inclusion in the referral guide.
