Project Title: The 2012 National Survey of Science and Mathematics Education

Investigator: Eric Banilower

Purpose: This study is designed to assess the quality of questionnaire items for the 2012 National Survey of Science and Mathematics Education. We want to identify items on the questionnaire that are confusing or difficult for teachers to answer, and to ensure that teachers interpret the items as intended. We are asking approximately 150 teachers to participate in this study.

Procedure: If you decide to be in this research study, your participation will involve (1) answering a set of questionnaire items and writing down some brief notes about your thoughts/impressions of the items, and (2) taking part in one 60-minute, audio-recorded phone interview about this same set of questionnaire items.

Potential Risks: We anticipate minimal risk associated with participating in this study. Responding to the questionnaire and interview questions may cause some anxiety. We will attempt to minimize any anxiety by maintaining participant confidentiality and conducting interviews over the phone, rather than face-to-face. No individual responses will ever be reported with respondent identities attached.

Potential Benefits: Participants in this study may not directly benefit from participation, but there may be benefits for the larger education field.  The 2012 National Survey of Science and Mathematics Education will provide an opportunity to gain a better understanding of the current status of science and mathematics education in the United States.

Cost to You: Beyond the time you spend participating in the study, there will be no additional cost to you.  We estimate that you will spend 10-15 minutes answering a set of questionnaire items and writing some brief notes, and 60 minutes participating in a phone interview.

Compensation to You: If you are selected to participate in this study and fulfill the study obligations (spending 10-15 minutes answering a set of questionnaire items and writing some brief notes about your thoughts/impressions, and participating in a 60 minute telephone interview), you will receive a $30 honorarium. Honoraria for the study will be sent within four weeks of fulfilling study obligations.

Confidentiality: The following procedures will be followed to keep your personal information confidential in this study. The data (i.e., questionnaire item responses and interview responses) provided by you will be kept under a code number rather than by name. Audio recordings of interviews will be kept on a secure server. Only Horizon Research, Inc. staff and consultants, all of whom have signed confidentiality agreements, will have access to the data.

Subject Rights: Your participation in this study is voluntary. You do not have to be in this study if you don’t want to be. You have the right to change your mind and leave the study at any time without giving any reason. However, by signing this consent form you acknowledge that you will receive an honorarium only if your study obligations are fulfilled.
