This survey can be used to assess an individual school’s present status in integrating applications of information technology into the curriculum or educational management process.  This instrument may also be used with school leaders to start serious discussion about future technology activity and planning.

When completed with the survey, evaluate the results of the survey.  These findings should serve as a foundation for establishing a Technology Implementation Plan for your program.
