This questionnaire provides information that will help your college counselor write an accurate, insightful, and supportive letter of recommendation for you. Please be candid. Information will be handled with care. We also need an activity list or resume from you. It does not have to be perfectly formatted, but should be thorough. You have been given instructions and a sample activity resume. You can also find this on the OES college counseling webpage. Your college counselor needs this questionnaire and your activity list by the end of summer.
