The information entered in this questionnaire will be used to create our 2011-2012 student directory. Please fill out the form even if you do not want your's or your child's infomation listed in the directory. If your child's name is not listed, they may be inadvertantly left off of invitation lists that may be guided by other parents using our directory. Please contact your child's teacher or their room coordinator if there is a special concern.
