Consent to Participate
You are being asked to participate in a research study:
The Experience of Being Diagnosed with Cancer in Lesbian, Gay, Bisexual and Transgender Patients.
The purpose of the study is gain knowledge regarding the experience of being diagnosed with cancer by gay, lesbian, bisexual or transgender (LGBT) people. The results of the study will be published in healthcare journals, websites and presented at appropriate professional meetings. The online survey should take about 20 minutes to complete. Your name or other identifying information is not being collected. All results will be reported by groups. We ask that you complete this survey only once. You cannot return to the survey once you begin. There are no foreseeable risks for participation. The benefit of participating is that your answers will add to the understanding of how to provide the best care to LGBT people who are being told of their cancer diagnosis.

You may also view the entire survey before completing any of it by clicking on this link: and a copy of the consent is included in the survey. Your participation in this research is voluntary, you may answer all or part of the questions. This study has been approved by the Institutional Review Board of California State University San Bernardino, San Bernardino California.
$50 dollar gift certificates are available in a raffle for people who complete the survey. Once you have completed the survey, send a separate email to info@cancer-network.org 212-675-2633 with your name and address with "raffle" in the subject line. Winners will be notified in April 2012.
