Survey of Skin and Scuba Divers in the December 2004 Indonesian Tsunami

This is a world-wide Internet survey of skin and scuba divers who were in the water in the Indonesian tsunami on 26 Dec 2004 . There has been little or no published information regarding the effects of natural disasters on divers. Drs. Tom Skalko and Carmen Russoniello of East Carolina University and I need your help in understanding what that experience was like for you as a diver and how it has affected your life since then.

Some of these questions may be upsetting to you, even months after the tsunami. Please take your time and do the best you can. It will take about 10 to 15 minutes to complete. This survey will automatically skip past questions that do not apply to you based upon your answers to earlier questions. Results will be compiled and completed as a group only. No individual identifying information will be released to anyone.
