Purpose: The purpose of this survey is two-fold. First, we would like you to help us by telling us how we can best allocate funds and resources to assist you in your emergency preparedness efforts. You will help us to know the right things to do by telling us your priorities and needs. Secondly, we would like to establish a base-line measure of healthcare facility preparedness to measure how we are progressing.

Confidentiality: All responses will remain strictly confidential. As you can see from the survey we are asking only minimal demographic information. No information from any one facility will be shared. Only reports in the aggregate and by region and by facility type will be shared. There will be no report that will show the results for any one grouping if it has less than 5 facilities.
Deadline: This survey will be posted on Wednesday, November 8, 2006. You are asked to complete the survey no later than December 31, 2006.
