Geriatric and Rural Interest Team Survey (GRITS)

Dear Colleague:

We would appreciate your input toward improving health care in Kentucky, particularly for vulnerable populations such as the rural elderly. Your responses to our five-minute survey of Kentucky primary care clinicians will help develop new projects pertinent to your practice and your patients. This survey is a joint effort of the Kentucky Ambulatory Network (http://www.mc.uky.edu/KAN) and the Kentucky Academy of Family Physicians (http://www.kafp.org). The Kentucky Ambulatory Network (KAN) is a statewide primary care practice-based research network.

Mammography screening for older women, age 65+ years, is a key health care concern for Kentucky. Therefore, the survey begins with items about mammography screening for older women. It then asks about other clinical and research concerns.

Participation in this survey research project is voluntary, and does not obligate you to future research projects. Your answers will be treated confidentially in any published reports or professional presentations. Your name on the survey will facilitate future collaboration. Data from the web-based survey wil be transmitted directly to a secure database. However, because third parties may intercept electronic data transmissions, we cannot guarantee private responses.
