To create an effective survey that gathers the information you would like to collect, it is important to choose your survey questions wisely. Multiple Choice Questions
- One Answer - Bullets: This question type allows the survey taker to select one option from a list of answers that you define. All answers are displayed on the screen at once. You can include an "other" option that will allow the survey taker to enter their own answer if none of the provided
answers applies to them. - One Answer - Drop Down: This question type allows the survey taker to select one option from a list of answers that you define. The answers appear in a pulldown menu.
- Multiple Answers - Bullets: This question type allows the survey taker to select multiple answers from a list of answers that you define. All answers are displayed on the screen at once. You can include an "other" option that will allow the survey taker to enter their own answer if none of the provided answers applies to them. We recommend that you include the text "select all that apply" as part of the question.
Rank Order Scales
This question type allows survey takers to rank items in order of preference by selecting a numeric value for each answer prompt. Each numeric value can be used only once. You can also include a "Don't Know" option if desired. Make sure to include the meaning of the scale in the question.
Rating Scales
- One Answer - Horizontal: This question type allows the survey taker to select a single rating for your question. Each button has a number and you can add labels above each number. The responses are presented sidebyside, which is ideal for comparativetype questions that require a scale such as "Poor/Excellent" or "Less Likely/More Likely". When filling out the answer options, select the number of options and then type in an answer label to correspond with each number.
- Matrix: This question type allows the survey taker to select ratings for several different aspects of a particular question. The question and rating scale appears at the top and then each aspect of the question appears below. This question type can be transposed to change the rating values to decrease instead of increasing. When filling out the answer options, select the number of options and then type in an answer label to correspond with each number.
Open Ended Questions
- Open Ended - One Line: This question type allows the survey taker to type in a short response.
- Open Ended - Multiple Lines with Prompt: This question type allows the survey taker to type a short answer to a series of related questions. A general question is often divided into separate topics, which appear on multiple lines with the responses.
- Open Ended - Comment Box: This question type allows the survey taker to enter a longform answer to a question.
Dichotomous Questions
This question type allows the survey taker to select "yes" or "no" in response to a question.
Demographic Questions
- Name and Address (US): This question type allows the survey taker to enter an address in standard United States format.
- Name and Address (General): This question type allows the survey taker to enter an address in a format compatible with US or international addresses. This question type also includes a field for an email address.
- Date: This question type allows the survey taker to select a date and time answer for your question.
